Revalidation button for GH ECA status page is currently broken

Hitting the revalidation button for GH requests is currently broken on production, meaning manual reruns can only be triggered by the webdev team via database manipulation. There appears to be a JS error on the page, which might be breaking the captcha and submission code, which uses custom JS.
