Misaligned atomics will now issue codes 5 and 7 if they otherwise pass the MMU. The rvfi_trap.cause_type field will be set to 3 to mark that codes 5 and 7 are for misaligned atomics.

Removed exception codes 4 and 6 for misaligned atomics.
