PMP Assert - Updates

This PR updates pmprvfi_assert to:

  • Be up to date with latest RVFI fixes.
  • Fix wrong use of rvfi signals.
  • Adds a couple new asserts.
  • Clear up a handfull of warnings and todos.
  • Plus some small misc fixes.

Test status:

  • Formal - Passes or undetermined.
  • ci_check - Passes
