Update Debug Vplan

This PR updates the vplan from the 40x to debug spec version 1.0.0 (including triggers) and adds it to 40s.

Note: For those working on debug testing atm, please link to coverage in the vplan while completing tests. NB! Can we really trust the old link to coverage? Should we 1) be safe and delete all of the old linkage and write it anew, or 2) save time by using the old linkage and only adding new ones?

We have had an internal review.

I've left the review comments intact in the file, as the even older review comments were also left intact previously and I found them useful when updating this vplan now. Let me know if anyone knows any good reason not to do this.
