Hotfix 1.0.1 patch to production

Fixes:

• Fix some issues with going back steps and data consistency
• Add requirement to membership level to submit
• Issue with scheduler that would cause too many calls to database

New features added:

• Added scheduled DB task to clean up inactive in-progress forms
• Ability to define reply-to header in emails
• Validation logic and enhanced data roll-backs on leaving step
