Fix output generator visibilities

  • Fix missing visibilities of OutputGenerator, IOutputGenerator, IEntityOutputPathHandler, OutputPathHandler
  • Cleanup usages of OutputGenerator and OutputPathHandler
  • Add default value for entity_output_path_handler in constructor of GtGenEnvironment
  • Move readCSV to Core/Tests/TestUtils
