Generated ExceptionsV coverage files in source directory

covergroupgen.py creates ExceptionsV_coverage.svh in the fcov/priv directory. This directory contains checked in files except the ExceptionsV files that are explicitly git ignored. Generated files should be kept in separate directories from source controlled files.
