Create working group application page in the Portal for existing members

To facilitate easier joining of working groups, a new page will be added to the membership portal to allow existing users to join working groups though a shortened version of the member application form. This will allow a user with a set of permissions to engage Eclipse Foundation through a form instead of through email. This builds on the work done in #630 (closed) and in #605 (closed).
