Email body needed for contact removal request

An email body needs to be defined for organization contact removal requests. This message should provide the context needed to initiate these requests, which are to be manual for phase 1.

@zfazli I believe you are the main person that will be receiving these requests. What information do you need for these requests? Do you need the requesting user details as well as the target, or just the person that is to be removed? Do we want to send a message to the user being removed, or just membership? Or is there another person in the org we should inform?
