I would avoid reinventing the wheel. EF have a well documented policy to contribute. I would refer to it. It is summarised here: https://www.eclipse.org/legal/ECA.php

Beyond this point, if we want to add mandatory declarations, fine, but first I would make a reference to such policy.

EF also have a "Get Involved" page: https://www.eclipse.org/contribute/ I would refer to it in order for new participants (contributors) to get an account and contribute.

On top of this, there is the wiki page refereed by @pcoval

Two more data points, in this case from Jakarta, a popular EF project:
