Investigate duplicate records for notices being generated by application

In testing with real data, it was discovered that there is some duplication of notice records being generated. Part of this seems to be due to some users having multiple active ECA documents, though there is more duplicate records than there are ECA documents, so I believe there is more to this bug than just that initial issue.
